Job Description

American Traveler is seeking an experienced Home Health RN Case Manager in Vermont with OASIS and case management experience.

Responsibilities

Home health setting providing patient care in the community
Day shift schedule, Monday through Friday with weekend rotation
Position is based in Vermont
Role includes case management responsibilities
Preference given to candidates with prior travel nursing experience
No requested time off accepted for the duration of the contract
Local candidates are accepted
Must be available for weekend rotation as scheduled

Requirements

Active VT or compact RN license
Minimum 1 year of home health nursing experience
OASIS experience required
Home health and case management experience required
BLS or BCLS certification required
Active license must be currently in hand
