Position Details

Experience Required: Minimum 1 year of recent Home Health experience
EMR: HCHB (HomeCare HomeBase) experience required
OASIS: Required
Average Patients/Day: 5–7 (to be confirmed during interview)
Types of Cases: Geriatric

Requirements

Certifications: BLS (AHA)
License: Active WI RN License (must be in hand)
On-Call: Yes, every 3rd to 4th weekend
Territory: Up to a 60-mile radius from the hub/office
Locals: Accepted at travel rate

Mileage Reimbursement: Yes (IRS standard rate)
Equipment Provided: iPad
Cell Phone Provided: No
Dress Code: Scrubs (any color) – No jeans
Special Notes:
- Traveler must be flexible with assigned territory.
