Description: Summary: General orthopedic surgeon needed for a 3-6 month assignment in Prescott Arizona to cover gaps of care prior to new providers onboarding.RATE CAPS FOR SUBMISSION:Malpractice Must be included in rates. Orientation 225 - half the hourly/callback rate kicks in after 8 hours. Per hour oforientation or training beyond 8 hours Hourly 450 - Per hour not OT Holiday 675 - Must be 1.5x the hourly rate. Per holiday hour no OT Nightly Call 1550 - Per night call to hold the pager to include 2.5 hours gratis Holiday Nightly Call - 2325 - Per night call to hold the pager to include 2.5 hoursgratis On Call Daily 3100 - Per 24 hours to include 4 hours gratis 24 Hour Holiday Call Rate - 4650 - Per 24 hours to include 4 hours gratisCall Back 450 Per hour ofpatient contact hours after gratis hours are completed gratis prorate to MSAterms. Holiday Call Back 675 Per holiday hour of patient contacthours after gratis hours are completed gratis prorate to MSA terms.
