Visiting Nurse Services in Westchester (VNS Westchester) is seeking dedicated and compassionate Physical Therapists (PTs) to join our growing home care team. Hiring Locations

We are actively hiring Physical Therapists (PTs) for home care opportunities in:

Westchester County, NY
Rockland County, NY
Bronx County, NY

Key Responsibilities

Assessment: Evaluate patients' functional limitations and develop personalized therapy plans with patients, families, nurses, and physicians.
Direct Care: Deliver therapy services using accepted physical therapy modalities for medical and surgical conditions.
Education: Train and supervise patients, families, and home health aides on exercise programs and care techniques.
Ongoing Evaluation: Monitor and adjust therapy plans as patients' conditions evolve.
Communication & Documentation: Report updates to physicians and managers, and maintain accurate clinical records of evaluations, treatments, and outcomes.

Qualifications

Active New York State Physical Therapist (PT) license & registration (required)
Minimum 1 year of full-time physical therapy experience
Home care experience preferred (not required)
Ability to use technology (tablets) for documentation; Oasis experience a plus
Valid driver's license and reliable automobile required for field visits

Compensation & Benefits

Salary: $95,000 - $108,000 annually (based on experience)
