Monday through Friday, 8 am - 4:30 pm schedule
Patient population age 55 and older
PACE program setting
Focus on chronic disease management
Expertise in cardiovascular and respiratory conditions required
Management of complex gastrointestinal disorders
Experience with geriatric-specific conditions needed
BLS certification required
