LVN Discharge Planner at Providence Little Company of Mary Torrance in Torrance, CA. This position is Per Diem and will work 8-hour, Day shifts.

The Care Management Assistant (CMA) collaborates with the patient/family, the acute multidisciplinary team, and post-acute providers to implement the patients discharge plan. The CMA also proactively escalates risks and barriers to discharge to the Case Manager; documents all discharge planning efforts in the patient's medical record in a timely manner; maintains ongoing communication with the multidisciplinary team and follows all regulatory requirements and ministry polices regarding care coordination/discharge planning.

Required Qualifications:
- Upon hire: California Licensed Vocational Nurse.
- 1 year Healthcare related position.
Preferred Qualifications:
- Associate's Degree Healthcare related.
