Job Description
American Traveler is seeking an experienced RN Case Manager with at least 3-5 years of case management experience, a BSN, and Epic proficiency for an adult patient population.
Responsibilities
- Work in a hospital case management department with an average daily caseload of 14-16 patients
- Care for adult medical/surgical, cardiac, and orthopedic patient populations
- Possible floating to any unit within the facility
- Day shift schedule with 8-hour shifts (0800-1700), five days per week; weekend shifts and alternative schedules may be assigned
- No on-call responsibilities
- Travelers are expected to work all holidays unless prior approval for time off is granted
- Uses Epic EMR exclusively; experience required
- Responsibilities include assessment, care coordination, discharge planning, and complex placement for varied needs including SNF, IPR, LTAC, hospice, home health, DME, behavioral health, and homeless populations
- Collaboration with a supportive interdisciplinary team including Directors, Managers, social workers, UM team, peer case managers, and providers
- Department culture is teamwork-oriented, communicative, and supportive with seasoned staff
- Departmental orientation lasts between 2-5 days based on skills and comfort level
- Scrub color is navy blue for RNs
- Free parking is available onsite
Requirements
- Active NC or compact RN license
- Bachelor of Science in Nursing (BSN) required
- Current BLS certification required
- Case Management certification preferred
- At least 3-5 years of recent case management experience is required
- Prior travel experience as an RN Case Manager required; first-time travelers will not be considered
- Epic EMR experience is required
