Job Responsibilities:
The MDS Coordinator ensures the accuracy of resident assessments and care plan optimization. To achieve this goal, this position's responsibilities include, but are not limited to:

Conducting and coordinating the development and completion of resident assessments (MDS) by current rules, regulations, and guidelines
Ensuring accurate and timely completion of the MDS process
Collaborating with the interdisciplinary team to develop individualized resident care plans
Monitoring and documenting resident progress and responses to care plans
Participating in the Quality Assurance and Performance Improvement (QAPI) process
Communicating with residents, families, and other healthcare providers to coordinate optimal care delivery
Staying updated on MDS regulations and guidelines to ensure compliance

Job Requirements

Current RN (Registered Nurse) license
RAC-CT (Resident Assessment Coordinator-Certified) certification is required
Strong understanding of MDS 3.0, RAI process, and care planning
Excellent organizational, communication, and interpersonal skills
Ability to work independently and collaboratively within a team
Commitment to providing high-quality care to residents
