Job Description
Job Title: Wound Care Coordinator
Profession: RN

Experience: Minimum 3 years of recent Wound Care Coordination experience
License: Must have an active RN license
Certifications: BLS certification, WOCN, CWS, or WCC certification
Must-Have:
- Experience managing wound care programs
- Proven track record in direct patient care and clinical consultation
- Strong educational skills for staff training
Description:
Manages facility wound care program including standards of care and practice related to wound, ostomy, and continence patient care needs.
Provides direct patient care and assistance to staff nurses.
Acts as a consultative service to affiliated healthcare agencies.
Develops and implements the facility wound care program in conjunction with the national standardization process, including patient care protocols and wound care formulary.
Establishes standards of care, competencies, policies, and procedures for effective wound care.
Acts as primary consultant to wound care clinicians.
Serves as a consultant to staff and physicians, providing training and support.
Enables clinical staff to assess wounds, recommend protocols, and initiate plans of care.
Provides ongoing education to staff on wound care products.
Evaluates all wounds upon admission and ongoing to determine treatment plan and problem identification.
Consults on patients with pressure injuries and develops plans of care in consultation with physicians.
Reviews medical records ensuring accurate documentation of wound assessments and treatments.
Investigates adverse events related to wounds through root cause analysis and develops action plans based on findings.
Participates in clinical outcome monitoring and performance improvement initiatives.
Collaborates with clinical operations staff to maintain a cost-effective wound care formulary.
Assists with training of new hire wound care coordinators within the facility.
