Location: Mantey Heights – Grand Junction, CO
Schedule: Full-time / Part-time
Reports To: Director of Rehabilitation (DOR) / Supervising Physical Therapist

Position Summary

The Physical Therapist Assistant (PTA) works under the supervision of a licensed Physical Therapist to provide rehabilitative services to residents. The PTA helps implement treatment plans, monitors resident progress, and assists residents in regaining strength, mobility, and independence.

Responsibilities

Carry out treatment plans established by the Physical Therapist.
Provide therapeutic exercises, functional mobility training, and physical modalities to residents.
Document resident progress accurately and in compliance with facility and regulatory standards.
Educate residents and families on exercises, mobility techniques, and safe practices.
Collaborate with the rehab team, nursing staff, and physicians to achieve resident care goals.
Participate in care plan meetings and contribute to quality improvement initiatives.

Qualifications

Current Colorado Physical Therapist Assistant license in good standing.
Graduate of an accredited PTA program.
Experience in skilled nursing or geriatric care preferred, but new graduates are welcome to apply.
Strong communication and interpersonal skills.
Dedication to teamwork and resident-centered care.
