Job Description:
Advocate Health is seeking a dedicated RN Home Health Case Manager to provide field nursing care to patients in their homes. Responsibilities include conducting patient visits, documenting OASIS encounters, and coordinating patient care to ensure optimal health outcomes.

Shift Details:

8-hour Day Shift (40 hours per week)
Monday-Friday, 8:00 AM – 5:00 PM
Weekend rotation with a take-back day
No on-call or call-back requirements
No holiday requirements
No floating required
