Pertemps Social Care are recruiting agency social workers for roles within a Hospital Team based in Huddersfield.

About the Role

You will be working as part of a multi-disciplinary team, assessing and facilitating safe and timely discharges with a commitment to promoting independence.

Responsibilities

Rapid Assessment under Discharge to Assess guidelines for hospital discharge
Assessing service users, mental capacity and making best interest decisions
Liaising with the discharge liaison team, as well as ward staff and family members.
Arranging care packages on discharge and step down beds.
Completing reviews and support plans.
Reporting any delays with service users.
