The Case Manager RN is responsible for care coordination and care transitions throughout the acute care patient experience. This role ensures timely progression and transition of patients to appropriate levels of care while preventing unnecessary admissions or readmissions. The Case Manager works collaboratively with physicians, utilization managers, social workers, bedside nurses, ancillary services, payers, and community resources.
Key Duties include:
- Performing initial and ongoing patient assessments, including psychosocial and financial needs.
- Leading interdisciplinary teams to create cost-effective, comprehensive care plans.
- Managing utilization reviews, ensuring documentation supports level-of-care requirements.
- Monitoring length of stay and facilitating timely discharges or transitions.
- Coordinating post-acute services, referrals, and regulatory documentation.
- Facilitating patient/family education and support for safe transitions of care.
- Actively participating in rounds, department operations, and quality improvement efforts.
- Maintaining compliance with CMS, Joint Commission, and state/federal requirements.
