Rehab RN needed to provide direct and indirect care for adult and geriatric patients in an acute rehabilitation setting. The nurse will manage patient care plans, coordinate with interdisciplinary teams, and support patient/family education to optimize recovery.
Key Responsibilities:
- Manage care for assigned rehab patients (typical ratio 1:5).
- Provide direct nursing care and supervise ancillary staff.
- Administer medications, manage IV therapy, ports, and feeding tubes.
- Perform wound care, suture/staple removal, and pressure ulcer prevention.
- Support patient mobility with lifts, transfers, and assistive devices.
- Provide diabetic teaching, infection prevention, and safety assessments.
- Collaborate with PT, RT, Pharmacy, Radiology, and Phlebotomy for integrated care.
- Float as required; may be assigned to additional units based on hospital needs.Ax
