Radius Requirement: Traveler’s permanent address must be at least 50 miles from the home unit
About the Role:
We are seeking a dedicated and experienced Home Health RN to join our team, providing care across multiple cities in North Carolina. This role requires flexibility, strong clinical skills, and a passion for delivering patient-centered care in a home setting.
Coverage Area & Travel Expectations:
- Service areas include: University City, Cornelius, Davidson, Kannapolis, Salisbury, Concord, Huntersville, and nearby cities
- No maximum mileage cap; average daily travel is 60–80 miles
- Weekly mileage reimbursement provided
- Ideal housing location: University City or Concord
- Must be comfortable floating throughout all service areas
Caseload & Scheduling:
- Required to complete 6 patient visits per day (including routine visits, discharges, recertifications, reassessments, and resumption of care)
- Nurses schedule their own visits with patients the day prior
- Cancelled appointments do not count toward visit equivalents; 30 visit equivalents (VE) required per week
Experience & Qualifications:
- Minimum Experience: 1 year of Home Health nursing
- First-Time Travelers: Accepted if experienced in Home Health
- Licensure & Certifications:
- Active RN license
- BLS certification
- Valid Driver’s License and proof of auto insurance
Patient Population & Common Diagnoses:
- Adults with chronic conditions
- Wound care and wound vac management
- IV therapy and pleurx drain care
Required Skills:
- Proficiency in Homecare Homebase (HCHB) – mandatory
- IV therapy, pleurx drains, wound vacs, wound care
- Strong assessment and disease education skills
- OASIS documentation
- Case management for 25–30 patients per episode
- Preferred: Experience with CADD IV pumps
Technology & Equipment:
- EMR: Homecare Homebase (HCHB)
- EPIC Rover (view-only access)
- Focura (for uploading discharge instructions and wound documentation)
- Samsung Note 9 phones provided for documentation and communication
Orientation & Onboarding:
- New Employee Orientation (NEO)
- 5-day precepted departmental orientation
