What You'll Do - Demonstrate commitment to NLCS' core values in all aspects of your work
- Support NLCS policies, procedures, guidelines, and code of conduct standards
- Cultivate an inclusive and supportive environment that empowers team members and residents to thrive and pursue personal and professional growth, in alignment with our philosophy of purposeful living
- Foster a positive work environment that promotes team member engagement and satisfaction
- Coordinate individualized plans of care using an interdisciplinary team approach
- Review and evaluate resident MDS assessments and care plans to ensure proper implementation
- Participate in resident care plan conferences
- Maintain the MDS schedule within required time frames and communicate pending assessments to relevant directors
- Generate and distribute the monthly MDS calendar
- Complete and audit MDS assessments, ensuring all required signatures are obtained
- Transmit electronic copies of assessments to the Centers for Medicare and Medicaid Services (CMS) and reconcile all transmissions
What We're Looking For - Active RN License in good standing
- RAC-CT Certification
- MDS Expertise: Proficient in completing and overseeing Minimum Data Set (MDS) assessments, ensuring compliance with federal and state regulations.
- Leadership Skills: Proven ability to lead interdisciplinary teams, facilitate meetings, and mentor staff in MDS processes.
- Analytical Skills: Excellent analytical abilities to assess data trends and implement effective performance improvement initiatives.
- Communication Skills: Outstanding verbal and written communication skills for effective collaboration with staff, residents, and regulatory bodies.
- Regulatory Compliance: Familiarity with Virginia state regulations related to nursing home quality assurance programs.
- Problem-Solving Abilities: Strong problem-solving skills to identify areas for improvement and develop actionable plans
