This role involves day shifts from Monday to Friday, 8 a.m. to 5 p.m., and is primarily focused on providing care in subacute rehab, long-term care, and dementia units.

Responsibilities and Duties
- Provide comprehensive medical care to elderly patients with various health conditions
- Manage and oversee the treatment plans for patients in subacute rehab and long-term care settings
- Diagnose and treat acute and chronic diseases prevalent in geriatric patients
- Collaborate with a multidisciplinary team to improve patient outcomes
- Utilize the PointClickCare EMR system for documentation and management of patient records
Additional Information
- Required Board Certifications: Family Medicine, Geriatric Medicine, Internal Medicine
- Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS) certifications required
- Must meet credentialing requirements, including a proficient reference from the most recent supervisory position
- No gaps exceeding 6 months on CV without a valid explanation
