Position Overview:
As a Home Health Occupational Therapist, you will:
- Evaluate Patients: Assess patients' physical, cognitive, and functional abilities to develop individualized treatment plans.
- Develop Treatment Plans: Create and implement therapy programs aimed at improving daily living and work skills.
- Provide Therapeutic Interventions: Utilize evidence-based techniques to enhance patients' independence and quality of life.
- Educate Patients and Families: Offer guidance on adaptive equipment and home modifications to ensure a safe environment.
- Collaborate with Healthcare Team: Work closely with physicians, nurses, and other therapists to coordinate comprehensive care.
- Document Progress: Maintain accurate records of patient evaluations, treatment plans, and outcomes.
What We're Looking For:
- Licensed Professional: Active license and registration New York State.
- Experience: experience in occupational therapy; home health experience is preferred.
- Communication Skills: Excellent interpersonal skills to effectively interact with patients, families, and team members.
- Reliability: Strong organizational skills and the ability to work independently in a home setting.
