✅ What You'll Do as a Home Health LPN

Provide direct nursing care to patients in their homes across assigned territories
Administer medications, wound care, IV therapy, and other treatments
Educate patients and caregivers on disease processes, medications, and home care techniques
Collaborate closely with RN Case Managers and the full interdisciplinary care team
Ensure accurate and timely clinical documentation

Enjoy a 32-40 hour/week schedule with flexible AM start times to support your personal and professional balance.

What You'll Need to Join as a Home Health LPN:

Active LPN license in the state of Massachusetts
At least 2 years of nursing experience (1 year of home health preferred)
Reliable transportation and a valid driver's license
Compassion, attention to detail, and commitment to patient-centered care
