Job Description
Job Title: Wound Care Coordinator
Profession: RN

Experience: Minimum 3 years in a wound care coordinator role
License: RN license required
Certifications: WOCN CWS or WCC certification required. BLS certification required through the American Heart Association.
Must-Have:
- Minimum 3 years recent wound care coordinator experience.
- Indiana RN license attached to resume.
Description:
Manage the facility wound care program.
Oversee standards of care and practice related to wound, ostomy, and continence patient care needs.
Provide direct patient care and assistance to staff nurses.
Act as a consultative service to affiliated healthcare agencies.
Develop and implement the facility wound care program in conjunction with national standardization processes.
Establish patient care protocols, documentation tools, wound care formulary, and referral criteria.
Create standards of care, competencies, policies, and procedures in quality, cost-efficient, and effective wound care for clinical staff.
Serve as the primary consultant to wound care clinicians.
Advocate for facility staff and physicians.
Enable clinical staff to effectively assess wounds, recommend appropriate protocols, and initiate plans of care.
Provide ongoing education to staff on products available for use in the hospital.
Evaluate all wounds upon admission and ongoing to determine treatment plans and provide early problem identification.
Consult on patients with pressure injuries.
Consult on wounds that do not show measurable signs of healing within two weeks.
Assist primary nurses in developing appropriate plans of care for comprehensive wound management and prevention.
Make recommendations to physicians for changes to wound care orders and support with evidence-based research.
Review medical records of patients with wounds.
Ensure wound assessments, care plans, and treatments are documented clearly and correctly.
Investigate cases of adverse events related to wounds through root cause analysis and develop action plans.
Participate in clinical outcome monitoring and performance improvement initiatives.
Collaborate with clinical operations staff to maintain a cost-effective wound care formulary.
Assist with the training of new hire wound care coordinators as needed.
