Care Manager RN - Case Management at Providence Petaluma Valley Hospital in Petaluma, CA. This position is Per Diem and will work 8-hour Day shifts.
Providence Petaluma Valley Hospital provides outstanding patient care which has garnered our placement on the Maternity Honor Roll for the California Health and Human Services Agency. Utilizing a collaborative process, the Case Manager will establish a continuum of care plan for a caseload of patients. The Case Manager will establish quality monitoring indicators, collection of data, analysis, documentation and the reporting outcomes, and liaison with patients. The Case Manager will act as a liaison to facilitate a smooth transition for the patient from acute to the next level of care.

Required Qualifications:
- Upon hire: California Registered Nurse License
- 2 years Acute care clinical or comparative experience
Preferred Qualifications:
- Upon hire: CCM
- 3 years experience in community health nursing or case management
