Job Overview:
We are currently seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team in Long Beach, CA. This is a full-time, permanent opportunity for a dedicated nursing professional who is passionate about providing high-quality, patient-centered care in a home setting.
You will work independently in the field while collaborating with a multidisciplinary care team to support patients in managing their health and improving quality of life at home.
Key Responsibilities:
- Deliver skilled nursing care to patients in their homes, following the established plan of care and physician’s orders.
- Perform comprehensive assessments, monitor patient progress, and update care plans as needed.
- Educate patients and their families on disease management, medication, treatments, and self-care techniques.
- Administer medications, perform wound care, and manage medical equipment as needed.
- Maintain accurate and timely clinical documentation using EMR systems.
- Coordinate care with physicians, therapists, and other home health professionals.
- Observe and report changes in the patient's condition to the appropriate healthcare provider.
- Comply with all applicable local, state, and federal regulations and policies.
Qualifications:
- Active California RN License in good standing.
- Minimum of 1 year of RN experience, preferably in home health or acute care.
- Current BLS Certification (American Heart Association).
- Valid driver’s license, reliable transportation, and auto insurance.
Preferred Qualifications:
- Previous Home Health or Hospice experience.
- Knowledge of OASIS documentation.
- Bilingual (e.g., Spanish) is a plus.
