**The right person for this job will be very proficient in Microsoft Office products. You will need to be personable, friendly and well-versed with effective communication, attention to detail and customer service.
Essential Functions:
- Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient care and planning for discharge.
- Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
- Supervises paraprofessional and licensed professional team members.
- Assures ongoing interdisciplinary assessment of the patient.
- Assures the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
- Assures services that are ordered by the physician are provided as indicated in the plan of care.
- Counsels and educates the patient and family in meeting healthcare related needs.
- Assures timely and accurate clinical notes including addressing patient's progress towards goals.
- Coordinates services including patient care and referrals (alongside Client Service Supervisor).
- Responsible for the Quality Assessment and Performance Improvement (QAPI) program.
- Responsible for the Emergency Preparedness training/exercises.
- Follows accepted standards of nursing practice.
- Uses clinical decision making to efficiently and effectively manage an assigned patient population.
- Accesses personal health information ("PHI") necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.
- Other duties and tasks as assigned by management.
Minimum Education & Experience Requirements:
- Licensed Registered Nurse in the State of South Dakota
- Graduate of an accredited school for applicable discipline.
- License is active and in good standing.
- Three (3) years home care experience preferred.
- Two (2) years management experience preferred.

- Monday-Friday
- 8:00am-4:30pm
- On Call
- Office setting
