Job Summary
The RN – Home Health provides competent clinical nursing care to patients in a home health setting, consistent with professional standards and policies. The nurse is accountable to the Nurse Manager/Director and is responsible for assessing, planning, implementing, and evaluating patient care to achieve optimal outcomes.
Essential Duties & Responsibilities
Patient Assessment & Care
- Recognizes data from complex situations to determine care priorities.
- Performs population-appropriate physical, psychosocial, educational, and safety assessments.
- Anticipates patient and family discharge needs and facilitates smooth transitions of care.
- Establishes therapeutic relationships with patients and families to support advocacy.
Planning & Implementation
- Collaborates with patients and families to develop and update individualized care plans.
- Organizes and prioritizes care according to patient/family needs.
- Performs bedside point-of-care testing and ensures accurate documentation in the EMR.
- Coordinates multidisciplinary resources to address complex patient needs and safe discharge planning.
Education & Communication
- Assesses patient/family readiness to learn and provides appropriate teaching using available resources.
- Communicates relevant patient information to promote continuity of care.
- Identifies cultural and developmental factors affecting care and patient education.
Leadership & Professional Development
- Participates in unit goals, performance improvement initiatives, and cost-effective care practices.
- Collaborates across departments to ensure safe and efficient patient care.
- Seeks opportunities for professional growth through continuing education and evidence-based practice.
Research & Quality Improvement
- Engages in self-directed learning, reviews relevant clinical studies, and contributes to discussions on practice applicability.
- Supports a safe, clean, and compliant care environment consistent with regulatory standards.
Required Qualifications
- Education: Nursing Diploma, ADN, or ASN from an accredited nursing program
- Experience: Minimum 1 year of nursing experience caring for acute and chronically ill patients
- Licensure: Current RN license in the state of North Carolina
Preferred Qualifications
- Education: BSN or MSN from an accredited nursing program
